What should be done if there is an issue?

If an issue arises during the operation, work must be stopped and the incident must be reported. If a breakage has occurred keep all fragments of the broken product, workpiece, etc., for examination by the product supplier and record operating details.
When stopping the equipment take into account that products should not be brought to rest by applying pressure to their surface and that hand-held and mobile machines should not be released or put down until the wheel has stopped rotating.
To report an incident, in the first instance, call the immediate supervisor and/or a member of the site team on-site. Raise the alarm or contact the emergency services if required. If the issue is an accident, the casualty should not be moved unless there is immediate danger until the emergency services are on-site.
When the danger is over, the incident should be reported in both, the site accident book and the company accident book.
However, always observe national regulations relative to reporting accidents and injuries.
Furthermore, it must be ensured that the machine involved in the incident is not used again until the cause of the problem has been determined and rectified.
